To help support you as you explore, develop and thrive in your interests, Forvis Mazars is pleased  to continue our Lifestyle Spending Account (LSA), LifeBalanced.  This benefit will reimburse up to $1,000 annually ($250 per quarter) for eligible wellbeing expenses in the following categories: Emotional, Physical, Financial, Tech, Convenience and Family Support. This means you can be reimbursed for the money you spend on items and services that help improve your overall wellbeing. Reimbursed funds are considered taxable wages but are not eligible 401(k) earnings.

This program is available to all benefit eligible Full-time and Part-time staff who work at least 20 hours per week (1,000 hours per year).  You do not have to be enrolled in benefits in order to receive the reimbursements.  This program is not available to Talent Shift, Interns, Seasonal employees, Managing Directors, Principals and Partners.

  • Newly hired employees will receive their first deposit for the quarter immediately following their date of hire. (Example:  Hire Date of January 15 would receive first credit deposit on April 1). Claims cannot be dated prior to their first quarter contributions.

Taxation of Lifestyle Account claims paid January 2026 – October 2026 will appear on your 2026 W-2.

Taxation of Lifestyle Account claims paid November 2026 – December 2026 will appear on your 2027 W-2.

You may submit claims with Benepass up to 14 days after your termination date for expenses incurred on or prior to your termination date.

Debit Card or Claim Reimbursement Process

  • You  may use your Benepass debit card at many retailers/vendors for immediate payments. Your LSA debit card will not work at certain big box retailers such as Target, Walmart or Amazon on-line.
  • You will only receive one debit card regardless of your enrollment in LSA, FSA and/or QTE.
  • The vendor code each vendor passes when a customer uses a debit or credit card will determine which account to apply the transaction to.  If you have multiple accounts with Benepass (LSA, FSA, QTE), it is recommended you periodically check your accounts.  If you notice a discrepancy, please contact Benepass at support.getbenepass.com or by clicking the ‘Contact Support’ button directly within your account.
  • Quarterly accruals of $250 will be deposited into your Benepass LSA account at the first of each quarter – January, April, July and October.
  • Newly hired or newly benefits eligible employees will receive their first deposit beginning the quarter immediately following their date of hire.  (Example:  Hire date of January 15 would receive first quarterly deposit in April)
  • You may use your debit card or request reimbursements at any time during the year; however, you may only use or be reimbursed up to the balance of your account.  If you attempt to use your debit card for more than your available balance, please understand your transaction may decline.
  • Funds used are considered taxable wages and will be treated as such on your W-2.

Claims Eligibility

  • The expense must be incurred during the current plan calendar year.
  • The expense must be incurred while you are a Forvis Mazars employee. Expenses incurred prior to your hire date or after your termination date will not be reimbursed.
  • Unpaid claims will not be reimbursed once you leave the firm, unless otherwise required by applicable law.
  • Reimbursement funds must be used in the current plan year as they will not roll over to the next plan year.
  • Upon termination, you have 7 calendar days to file a claim with Benepass for available LSA funds. These expenses must have incurred on or prior to your termination date.
  • Any funds remaining after 7 calendar days after the termination date will be forfeited.

Ineligible Expenses

Ineligible expenses include, but are not limited to:

  • Charges paid under another pre-tax benefit (doctor visits paid under insurance and/or HSA/FSA card for example)
  • Alcohol
  • Legal or illegal drugs or drug paraphernalia
  • Weapons of any kind (guns, including bullets, knives, etc.) 

  • Sports betting or other gambling or apps
  • Restaurants
  • Tobacco products
  • Any other expenses that Forvis Mazars firm leadership determines are inconsistent with their core values, policies or overall intent of advancing the wellbeing of all eligible Forvis Mazars employees.

Expenses not specifically listed above for reimbursement will be up to the discretion of firm leadership.

Simply run your card as credit for any eligible products or services. Keep documentation (an explanation of benefits, or EOB, typically has the IRS-required information) in case we need them to approve your charges.
