Adoption & Surrogacy Assistance

Helping You Build Your Family

This policy is designed to assist adoptive parents with the financial aspects of adopting a child or using surrogacy to welcome a child into their family.  This policy does not apply to the adoption of stepchildren.

Eligibility

  • Full-time and part-time employees that work a minimum of 20 hours per week.
    Interns, seasonal employees, temporary, term consultants, and partners are not eligible for this policy.

You also must be employed by the firm on the date payment is made.

Amount of Financial Assistance

Forvis Mazars’ qualified adoption and surrogacy plan will reimburse up to $10,000 in actual expenses for legal and adoption and surrogacy fees associated with the adoption and surrogacy of a child. The benefit plan is non-taxable.

Procedure

  • Complete the Adoption Assistance Request form
  • Submit form, copy of adoption and surrogacy expenses, birth certificate(s), and a copy of the legal documents that accompany the adoption and surrogacy to Workday
    • In Workday navigate to Actions → Compensation → Request One-Time Payment for Myself
